PADI Emergency First Response Instructor (EFRI)

EFR emphasizes positive coaching and guiding students to success without causing embarrassment. Stopping the class or failing them immediately violates this philosophy, while doing it for them doesn’t allow for proper student skill mastery.

According to EFR General Standards and Procedures, the maximum ratio is 12:1 during skill practice. Ratios of 8, 10, or 16 without appropriate assistants do not align with the standard EFR instructor guidelines.

Independent study is the standard EFR method for reducing classroom time while ensuring all knowledge development requirements are met. Omitting required skills, skipping scenarios, or removing individual practice violates EFR standards.

EFR Instructors must specifically complete the Care for Children instructor module and possess the required pediatric training aids. It is not an automatic qualification from Primary Care, nor does it require external medical credentials.

EFR providers are lay rescuers whose role is to provide basic care and relay information to emergency medical professionals. Diagnosing and prescribing medication are outside the scope of lay rescuer training and EFR standards.

EFR standards mandate a minimum ratio of one adult manikin per four students to ensure adequate practice time. Infant manikins are for Care for Children, oxygen is for a separate course, and a 1:2 AED ratio is not mandated.

EFR emphasizes a positive, self-guided debriefing process that builds confidence and reinforces learning. Harsh critiques, ignoring the situation, or forcing immediate repetition without reflection undermine the educational environment.

The final exam is an evaluation tool to ensure knowledge mastery and provides an opportunity for the instructor to remediate misunderstandings. It is not designed for ranking, simulating stress, or evaluating physical skills.

Current ILCOR/EFR guidelines require a compression rate of 100-120 per minute. The instructor must use positive coaching to correct the rate without failing the student immediately or switching to inappropriate care.

EFR Instructors must renew their teaching status biennially (every two years) to ensure they are up-to-date with current standards and guidelines. Annual co-teaching or lifetime validity are incorrect, and retaking the course is not required for standard renewal.

Moisture on the chest can prevent pad adhesion and cause the shock to arc across the skin rather than penetrating the heart. Drying the chest is a standard step in AED application, and waiting for sterile gauze or EMS is unnecessary and potentially fatal.

Skill development isolates specific techniques (like CPR or bandaging) for mastery. Scenario practice brings those skills together, teaching students how to manage an emergency from start to finish in a simulated environment.

Removing a blood-soaked dressing can tear away developing blood clots and worsen bleeding. Instructors must ensure students understand the principle of adding bulk to a bleeding wound rather than replacing dressings.

EFR courses are performance-based, not age-based. As long as a participant can physically perform the skills and understand the concepts, they can be certified, regardless of their specific age.

For pregnant or obese individuals, abdominal thrusts are contraindicated. The correct EFR and ILCOR protocol is to use chest thrusts to clear the airway obstruction safely.

EFR carefully selects terminology to build rescuer confidence. ‘Patient’ denotes someone being helped, whereas ‘victim’ implies helplessness and tragedy, which contradicts the proactive empowerment of the EFR course.

Pediatric cardiac arrests are primarily asphyxial (respiratory) in nature, whereas adult arrests are typically cardiogenic. Therefore, standard pediatric protocols prioritize early ventilation to address the most likely cause of the emergency.

While EFR is a prerequisite for PADI Rescue Diver, it is a standalone certification program. It must be taught using its own standards, materials, and exams, and cannot be informally merged into dive training without meeting all specific EFR requirements.

While elevating the legs is a standard treatment for shock, it is contraindicated if it causes pain or if there is a suspected spinal or leg injury. Administering medication is outside the lay rescuer’s scope, and the recovery position is not indicated for this specific situation.

The Cycle of Care emphasizes continuous monitoring and support of a patient’s vital functions (lifeline) until professional help takes over. It does not replace EMS, nor does it require complex medical terminology or encourage abandoning the patient.
